Q: What does it cost ?
A: FAMILY’s home visits are free to all families in Pottawattamie County. There is an application process and certain criteria to determine eligibility.

Q:How are you funded?
A: FAMILY receives funding from donations and grants. Some of our funding partners include Boost4Families, Delta Dental, Iowa Department of Education, Iowa Department of Public Health, Iowa West Foundation, Pottawattamie County Community Foundation, Delta Dental of Iowa Foundation, Medica Foundation, Healthy Tomorrows Partnership for Children, and Early Head Start.

Q: How do I enroll in the home visitation program?

Q: How do I enroll in the maternal health program?
A: Contact Cassy Lawrence, our Maternal Health Nurse at 712-828-2135.

Q: How do I know if my child is on track?
A: FAMILY offers a variety of child development screenings. To learn more please feel free to visit http://www.easterseals.com/site/PageNavigator/ntlc10_mffc_homepageasq.html to complete an online questionnaire to assess your child’s development.
